Fred J. CerwonkaOCTOBER 31, 2008 - Mr. Fred J. Cerwonka, Jr. 60, of Fernandina Beach, Florida passed away Thursday, October 30, 2008 at Baptist Medical Center-Nassau. Mr. Cerwonka was
He served in the United States Army during the Vietnam War and during his career was employed as an alarm system technician working for over thirty years before his retirement in 2000. Locally he was an active member of the American Legion Post # 54 in Fernandina. He always loved spending time at the ocean, taking walks on the beach, and was an avid fisherman all of his life. He also was an accomplished cook who enjoyed creating special dishes for his family. Survivors include his wife Erin E. Cerwonka, son, Freddie J. Cerwonka, III and nephew, Steve Nicklas (Jill) all of Fernandina Beach, Florida, sister-in-law, Florence Nicklas of Southampton, New Jersey and also by a number of nieces and nephews. The family will hold a private celebration of his life at their home. He will be laid to rest with military honors at Jacksonville V.A. National Cemetery at a later date. OXLEY-HEARD FUNERAL DIRECTORS Evelyn "Jeanne" BurkettOctober 30, 2008 - Mrs. Evelyn "Jeanne" Burkett, age 51, of Fernandina Beach , passed away on Wednesday afternoon, October 29, 2008 at her home. Born in Jacksonville , Florida , she had been a lifelong resident of Fernandina Beach . Mrs. Burkett was a graduate of Fernandina Beach High School , Class of 1974, and Florida Junior College , School of Nursing . She enjoyed reading, the beach and spending time with her family. She had worked as a Registered Nurse for many years at Memorial Hospital in Jacksonville . She was a member of the Memorial United Methodist Church , Fernandina. Mrs. Burkett is preceded in death by: her father, James "Jim" Malone and a brother, Jimmy Malone. She leaves behind, her mother, Irma Malone, Kingsland, GA, two daughters, Michelle Burkett, Marissa Burkett, both of Fernandina Beach, a brother, Mike Malone ( Maria), Jacksonville, FL, twin sisters, Patricia Rumptz, Kingsland, GA, Elizabeth "Peggy" Baker (Bill), Las Vegas, NV, three grandchildren, James, Eythan, Jayden and several nieces and nephews. Funeral services will be at 2pm, on Sunday, November 2, 2008 in the Burgess Chapel, with Reverend George Young, officiating. Mrs. Burkett will be laid to rest in Bosque Bello Cemetery . Friends and guests are invited to visit on Saturday from 10:00 -12:00 pm at the funeral home. "Jim" Duccilli, III and Steven Duccilli (Jackie) both of Cincinnati, Ohio, and Robert Duccilli (Irene) of Fairfield, Ohio, two daughters, Janet Daniel (Mike) of Fairfield, Ohio and Patti Ash (Jerry) of Wyoming, Ohio, sister, Sister Ruth Ann Bange, SND, Joseph (Phyllis) Bange, Dorothy (Stanley) Duke, Donald (Margie) Bange, and the late Father Ralph Bange and Mary (Hal) Fish. She was "Proud Gran" to her twelve grandchildren and is also survived by her numerous nieces and nephews. A Mass of Christian Burial will be held at St. James of the Valley Catholic Church, 411 Springfield Pike, Wyoming, Ohio. She will be laid to rest in Gate of Heaven Cemetery following the services. Forrest, age 63 of Fernandina Beach, FL passed away Thursday evening, October 23, 2008 at Baptist Medical Center/Nassau. Mr. Forrest was born on December 19, 1944, the son of the late Bruce and Madia Forrest in South Philadelphia, PA. He was a graduate of Claymont High School in 1962 in Claymont, DE. A step-daughter Jennifer (Billy) Watters of Costa Rica and a step-son Jason (Dee Dee) Lockwood of Lakeland, FL. Two brothers; Scott Forrest of Wilmington, DE and Donald Forrest of San Francisco, CA, five grandchildren; Elle, Emre, George, Shelby, and Taylor. And one special Mr. Peepers, his redheaded son the cat. Funeral Services with military honors will be held at 3:00 PM in the Burgess Chapel of Oxley-Heard Funeral Home on Sunday, October 26, 2008 with Rev. Jim Tippins officiating.
